Responsibilities

The primary responsibility of the LTAC Clinical Transition Coordinator (CTC) is to facilitate a seamless transition for patients discharging from a facility setting to the care of an LHC Group agency|facility for post-acute care needs. The LTAC CTC will respond to referrals and determine the level of care required by the patient upon initial evaluation. Following identification of new LTAC patient criteria, the LTAC CTC will complete a full risk-assessment of the patient, including a cost indicator using the net reimbursement worksheet. In addition, the LTAC CTC will implement re-hospitalization reduction initiatives for patients with Acute Care Hospitalization risk and continually communicate between healthcare providers during all phases of transition from the facility to the next level of care. The LTAC CTC establishes best practice intervention and education to improve patient outcomes and promote patient self-management upon discharge from the LTAC to the next level of care. The LTAC CTC role is clinically focused and aligned with company market development initiatives. This clinical liaison understands their role in growth, serving more patients and delivering exceptional care across the entire post-acute care continuum. Using discretion and independent judgement, the LTAC CTC identifies specific referral target audiences and develops referral-building strategies in collaboration with the Administrator. The LTAC CTC will interact with all departments and services as needed to complete the referral|evaluation process and address barriers to admission by working effectively with referral sources and the Admissions Team.



Education and Experience

Formal Education: Associate Degree or certification equivalent

Experience: 1 year

License, Registration, and / or Certification Requirement: Yes

Experience Requirements

  • One (1) year acute care hospital

License Requirements

  • Must have independent means of transportation, possess a valid driver's license and the ability to
  • Automobile LIABILITY insurance coverage must be
  • Current RN, LPN/LVN or RT license in state of practice or other professional licenses as applicable

Skill Requirements
  • Must have computer skills with knowledge of spreadsheet and word processor
  • Clerical and business machine skills and knowledge of business
  • Good organizational and personal communication

Skill Desired
  • Understanding of LTAC Industry
